Job Summary/Description:

The Mental Health Support Services Manager provides clinical supervision and coaching to an expanding, dynamic, dedicated team of Adult Behavioral Health Targeted Case Managers (Rule 79), as they advocate for people living with the challenges of severe and persistent mental illness. The Program Manager, along with the Team Leads, provides day-to-day leadership of the team and program management. They offer support and guidance to Case Managers, as they work with clients to improve or maintain functioning across multiple domains. The Program Manager will also carry a small client caseload, and lead the team in inter-agency collaborations and our partnership with Hennepin County.

Along with the Mental Health and Senior Services Director, the manager creates and develops opportunities for program growth and sustainability, including identifying opportunities to expand services to support more people with their mental health needs, beyond the provisions of Rule 79. The manager is responsible for defining clear goals and measures of success, collecting and analyzing data, evaluating outcomes, and developing and implementing strategies for continuous improvement.

Primary Duties and Responsibilities:

Essential Functions/Approximate Time:

  • Leadership in Staff Supervision: Provide supervision and leadership for all Targeted Case Managers and Team Leads. Monitor and manage workloads and workflow. Cultivate staff engagement, and lead regularly scheduled team meetings that foster group collaboration. Promote and model frequent feedback, performance development coaching, and holding staff accountable for meeting performance expectations. Participate in the recruitment, hiring, onboarding, and training of new staff. 35%
  • Leadership in Program Management: With support from program staff, define clear goals and measures of program success, collect and analyze data, evaluate outcomes, audit preparation, and develop and implement strategies for continuous improvement. Lead department staff in implementing the JFCS strategic plan. Identify and cultivate opportunities for sustainability and innovative growth through enhanced, new or expanded programs and models for case management. Serve as a liaison to other agencies and community groups helping to cultivate JFCS as a top-of-mind, trusted, top-quality service provider, thought leader, and strategic partner. 35%
  • Direct Service: Carry a caseload of up to 10 clients. Assess clients’ bio-psycho-social, vocational, and economic strengths, risks, and needs. Visit clients in their homes and/or community on a monthly basis. In collaboration with clients, develop and monitor comprehensive services plans with measurable goals. Facilitate access to necessary services. Advocate for clients, initiate appropriate referrals, coordinate ongoing services, and monitor client progress toward goals. Maintain accurate and timely clinical records and court documentation. Attend team, agency, and community meetings. 30%

Experience and Qualifications

Education and Licensing

  • Minnesota professional license at the LICSW, LP, LMFT, or LPCC level (alternatively, master’s degree in social work + 4000 hours of supervised clinical experience in the treatment of mental illness)
  • Approved supervisor for MN Board of Social Work and MN Board of Behavioral Health and Therapy preferred

Experience

  • Minimum of two years of experience in case management for adults with severe and persistent mental illness (SPMI)
  • Minimum of two years of experience hiring, training, and providing clinical supervision to multiple staff and/or students
  • Experience conducting assessments and/or clinical interviewing
  • Knowledge and experience with client confidentiality practices, HIPAA and electronic health record practices
  • Working or otherwise developing relationships with people from diverse backgrounds, such as differences related to culture, religion, financial resources, race, national origin, age, gender, gender identity and expression, sexual orientation, and abilities
  • Monitoring, evaluating, and reporting on outcomes

  • Proficient user of electronic health record systems for documentation and communication
  • Proficient user of Microsoft Office programs, including Word and Excel 
  • Current Minnesota driver’s license, safe driving record, auto insurance, and regular and consistent access to a motor vehicle
  • Ability to manage a hybrid work model between working in the office/field and working from home

Typical physical/mental demands:

  • Indoor office environment for most work, at JFCS and in the community. Combination of walking, sitting, and standing in an office setting. Frequent written and oral communication. Occasional lifting, stooping, kneeling, bending or climbing. Works with and around others. Daily use of computer, telephone, and other office equipment. Occasional evenings and weekends. This position requires transportation to participate in meetings and other appointments in the community.
  • Awareness of and ability to manage time to meet deadlines and complete work. Frequent interruptions and need to handle multiple tasks simultaneously. Basic computer skills (Word, Excel, email, and case note data entry and oversight). Occasional lifting of up to 15 pounds.
