 Community Chaplaincy

Our community chaplain is a Rabbi with additional training that includes Spiritual Care Emergency Response. Rabbi Lynn Liberman is available via telephone to offer support and connection. She will listen, provide insights from the Jewish tradition and acknowledge the challenges being faced by each of us at this time.  Community Support Program

We have launched this new program that includes three 20-minute phone conversations with one of our mental health professionals. The goal of the program is to help people feel more grounded and emotionally secure. The therapist will also discuss strategies for improving their sense of wellbeing.  Connection to Community Resources

There are numerous community resources available to ease your burden, both within JFS and elsewhere. JFS will help you identify and access the resources you need.  Counseling & Mental Health Services

JFS therapists specialize in the assessment and treatment of a wide range of mental health issues. Counseling is covered by a wide variety of health insurance plans. A subsidized sliding fee based on family income is also available for those without insurance. Contact our main number at (651) 698-0767 or by email at [email protected] to schedule an appointment. This is a fee-based service; however, we offer a sliding fee scale for those who qualify.

Emergency Financial Assistance

We offer emergency financial assistance to those who qualify. This assistance helps with emergency expenses related to housing, utilities, transportation and some health expenses. Kosher Meals on Wheels

Could you or someone you love benefit from a nutritious kosher/halal meal delivered to their home? Kosher Meals on Wheels provides this service in our community. The cost is based on income, and many participants qualify for free meals through their health plan. There is no age requirement for participation. PEARLS (Program to Encourage Active & Rewarding Lives) and PEARLS for Caregivers

PEARLS offers eight FREE one-on-one video streamed support sessions for older adults and those who provide care. The skills and tools offered in the program help participants address life’s problems and make ongoing plans to engage in a variety of pleasurable, meaningful activities. Additionally, we are contacting all former PEARLS and PEARLS for Caregivers clients to offer emotional support. PEARLS lead counselor, Avi-Natan Zadaka, LGSW and his colleague Tara Burns, MA will call to offer four supportive phone or telehealth contacts. Depending on individual needs and preferences, these sessions can range from 15 to 60 minutes each and occur once or twice monthly.  Former PEARLS clients can contact Avi at (651) 410-7822. Senior Care Services

Care Planning

Care Planning helps families notice changes in parents and/or spouses and helps families articulate issues and challenges, learn about available resources and create a plan for maximizing independence.

Care Coordination

Care coordinators become the primary coordinators for the delivery of multiple services including accompanying to medical appointments; following the client through hospital, nursing home, and transitional care unit (TCU) stays; and providing reports to family members.

Caregiver Coaching

Our caregiver coaches work with family members to guide them to improve everyday life at home; whether by promoting better self-care or by offering tips to deal with diagnosis-related behaviors. We can also work directly with people in the early stage of their disease to plan for their own future.

Advance Care Planning

Advance care planning is making decisions about the healthcare you would want to receive if you become unable to speak for yourself. Put into writing what types of treatment you would or would not want, a living will, and who you chose to speak for you as your health care power of attorney.

We are sharing a historic moment together and, although we are asked to be socially distant, our experience is now more connected than ever. We need each other.

The impact from COVID-19 has created new and increased needs. Almost overnight, we had to transform our service model from in-person contact to phone and video service. Counseling and support services, requests for Kosher Meals on Wheels and the desire for greater spiritual connection have all grown.
