Job Summary/Description:
The Registered Nurse (RN) Case Manager, Home Care (a) ensures that the health and safety of all Home Care clients is provided through development, implementation and evaluation of individual client care plans; (b) communicates personally with clients, staff and family members; and (c) assists in marketing the Home Care program.
Primary Duties and Responsibilities:
- Complete individual and regular assessments of clients to assure their needs will be met.
a.Implement and evaluate individual client plans of care.
b.Communicate to physicians and other appropriate personnel (e.g., county caseworkers) changes in client needs or conditions.
c.Initiate ongoing communication with clients, families, and responsible parties.
d.Assure all physicians’ orders are implemented within 24 hours.
e.Maintain an ongoing responsibility for assigned caseload.
- Supervise staff and participate in recognition, counseling, and corrective action, as needed.
a.Provide education to staff to meet Minnesota Home Care Rule and Statute requirements as needed.
b.Ensure that statutes and rules set forth by Minnesota Home Care licensure are met.
c.Develop appropriate communication tools for staff to ensure best quality care for clients.
d.Monitor and ensure that staff follows Sholom safety guidelines in all duties and responsibilities.
e.Monitor and ensure that medication assistance and administration procedures are followed properly and discuss any needed errors or corrections with staff.
- Assure clients maintain admission criteria; reassess and recommend alternative placement if necessary.
a.Implement up-to-date and individualized service plans/agreements based on assessments.
b.Monitor and maintain client charts and clinical progress notes.
c.Perform supervisory visits of clients.
d.Facilitate coordination of client services with other service providers.
- Ensure staffing needs are met in collaboration with the Staffing Coordinator.
a.Participate in the interviewing and hiring process of unlicensed personnel.
- Stay current on policies and procedures, and ensure compliance.
- Identify and respond appropriately to emergency situations, such as falls, injuries and storm-related evacuations.
a.Monitor for and address any safety hazards that exist in staff’s or clients’ habits and activities of daily living, and in the building environment.
SECONDARY RESPONSIBILITIES
- Perform miscellaneous projects and completes various tasks as requested by management.
- Be available on call.
Experience and Qualification Requirements:
Education, Experience and Credentials
- Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.
- Two years’ previous related experience in a similar position and in a similar care environment preferred.
- Current CPR certification and maintenance of certification preferred.
- Knowledge of medicare regulations and OASIS.
- Valid Minnesota drivers’ license with good driving record, and proof of valid insurance and/or have reliable transportation.

- Able to effectively present information in one-to-one and small group situations to clients, staff, and other customers.
- Must be able to perform the duties of unlicensed staff.
- Assist residents with mobility needs such as transfers, ambulation, escorts or use of assistive devices. Staff will follow established safety protocol and use proper body mechanics.
- Able to stay current on Sholom policies and procedures and attend in-service training as appropriate.
- Follow and adhere to Universal Precautions and Infection Control procedures.
PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB
Incumbent must be able to:
- Perform medium work, lifting up to 50 pounds, and frequently lifting/carrying of objects weighing up to 20 pounds.
- Push/pull clients and wheelchairs up to 50 pounds a distance of three feet.
- Sit at a desk for extended periods of time and perform long hours of work sitting at a computer.
- Frequently stand and walk during an eight-hour shift.
- Apply manual dexterity for computer keyboarding and office equipment use.
- Turn, bend, stoop, kneel, squat, and reach frequently in the performance of nursing responsibilities.
- Possess visual and reading ability for close-up work, including computer work, reading detailed documents, charts and/or reports.
- Effectively verbally and orally communicate in the English language, including following oral and written instructions.
- Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.
TOOLS AND EQUIPMENT USED
Incumbent must be able to use client assistive equipment, walkers, wheelchairs, electric wheelchairs, multi-line telephone, calculator, copy machines, computers, and medical equipment such as thermometers, blood pressure equipment, oxygen tanks, etc.
WORK ENVIRONMENT
The work environment is indoors. Occasional exposure to a variety of weather conditions in traveling outside of the workplace.
