Job Summary/Description:
The Targeted Case Manager provides direct case management service to adult clients with mental illness and their families. It assesses the client’s needs and develops and monitors comprehensive treatment plans with measurable short and long term goals. The position assists clients to live independently and successfully in the community while managing their mental illness, makes referrals to community resources, and advocates for and mobilizes resources for the clients when needed.
Primary Duties and Responsibilities:
Assess clients’ bio-psycho-social, vocational, economic, and educational risks and strengths.
In conjunction with clients, develop and monitor comprehensive treatment plans with measurable long and short term goals.
Link clients and their families to community resources, and advocate on behalf of clients for services not readily available.
Maintain accurate and timely clinical records, correspondence and reports as required by JFCS.
Attend supervision, unit and agency staff meetings and in-services.
Experience and Qualification Requirements:
EDUCATION: Bachelor’s degree in social work or human services related field.
EXPERIENCE: Two years of social service/case management with adults with mental illness. Experience working with people of various ages and backgrounds.
SKILLS: In-depth knowledge of the causes and treatment of mental illness, case management practices, community resources, and the Jewish community. Strong time management and organizational skills. Effective oral and written communication skills.
