Position Summary:

The Campus Director of Social Services provides leadership, vision and management to the Social Services department to provide social services to apartment tenants, nursing home and assisted living residents. Additionally, the Campus Director Social Services shares a responsibility toward creating and sustaining an environment that humanizes and individualizes each resident, working to promote their physical, social and psychosocial well-being.

Primary Duties and Responsibilities

  • Direct the operations of the Social Services department including the supervision and development of Social Services staff.
  • Lead and manage the Social Services department, participating in performance management and corrective action when necessary.
  • Participate in the interview and hiring process to ensure quality services are provided.
  • Organize staff education and ensure department compliance with applicable regulations.
  • Provide direct clinical and evaluation services for assigned caseload, providing social work services to residents/tenants as needed, including but not limited to assisting them manage their daily lives, cope with issues and navigate relationships.
  • Assist the resident/tenant in recognizing problems or concerns and develop adequate approaches to address these issues.
  • Assist with discharge planning for homeward bound residents.
  • Ensure documentation is maintained to comply with applicable regulations.
  • Monitor residents /tenants’ psychosocial status; evaluate goals and initiate changes in care plan accordingly.
  • Work closely with internal departments to ensure adequate flow of services and to best meet the needs of residents.
  • Establish and coordinate procedures with care teams to expedite direct services to clients.
  • Attend various meetings as required including Director Meetings, community, resident and family counsel, etc.
  • Set productivity and profitability objectives, create strategic plans, develop and manage budget and financial processes company-wide.
  • Establish and build mutually beneficial relationships with vendors.
  • Serve as a resource to staff within the care center, assisted living, and other internal and external sources.
  • Respond to and process referrals for services, admission, transfers, etc. from a wide variety of internal and external sources.
  • Assist with various other Social Service duties as needed.

Experience and Qualifications Requirements:

Education, Experience and Certifications

  • Must be licensed by the MN Board of Social Work and maintain licensure.
  • Three years or more of social work experience required, preferably in a long-term care environment.
  • Three or more years’ experience leading and supervising social services staff preferred.
  • Bachelor’s degree in Social Work preferred.

Knowledge, Skills and Abilities

  • Flexible and productive in an environment with regular interruptions.
  • Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.
  • Possess effective problem solving skills.
  • Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.
